You will need your treatment records for the ten patients you have seen most recently.  Once you've got these to hand, submitting your contribution should take no more than 30 minutes.  Your individual contribution will be treated confidentially and your name will not be used in the published results of this study.

 

When you are ready, please complete and submit this form all in one go.  Your contribution will be e-mailed to us when you click 'submit' at the bottom of the page.  Your input will not be saved on this website.  If you complete the form and for some reason are unable to submit it, please print a paper copy then contact us for our postal address.

Please don't be concerned if you feel these treatments are not 100% representative of your normal practice.  This will be averaged out across our study, since your results will be part of a much wider pool of treatments.  For our results to be reliable it's important that that everyone uses their last ten treatments accurately!

Needling a point on both sides of the body counts as 1.  The maximum you should put in any box is 10, which would show that you used that point during all ten of the selected treatments.

 

 

For example, if during your last ten treatments you used:

- Liver 1 not at all

- Liver 2 during one treatment

- Liver 3 during six treatments
